Liability Waiver

In consideration of being allowed to participate in any activities, programs, or services provided by Tyler Welch Performance Training, I, [Participant's Full Name], hereby acknowledge and agree to the following terms:
 

1. Assumption of Risk: I understand that participation in physical fitness training, exercise programs, and other related activities involve inherent risks of injury. I voluntarily assume all risks associated with my participation, including but not limited to, muscle soreness, sprains, strains, fractures, and other potential injuries.
    

2. Physical Condition and Medical Clearance: I acknowledge that I am in good physical condition and have no known medical conditions or disabilities that would prevent me from safely participating in the activities offered by Tyler Welch Performance Training. If I have any concerns about my health or ability to participate, I will consult with a medical professional and obtain necessary clearance.
    

3. Release of Liability: I hereby release, waive, discharge, and covenant not to sue Tyler Welch Performance Training, its trainers, employees, contractors, and affiliates from any and all liability, claims, demands, actions, or causes of action arising out of or related to my participation in the activities, including negligence or any other fault of the released parties.
    

4. Indemnification: I agree to indemnify and hold harmless Tyler Welch Performance Training, its trainers, employees, contractors, and affiliates from any and all liabilities, costs, expenses, or damages arising from my participation in the activities, including but not limited to, personal injury, property damage, or any other harm.
    

5. Photography and Video Release: I grant Tyler Welch Performance Training the right to photograph, record, and use my likeness in promotional materials, social media, and other marketing channels. I understand that my personal information will be handled in accordance with the privacy policy of Tyler Welch Performance Training.
    

6. Severability: If any provision of this liability waiver is deemed invalid or unenforceable, the remaining provisions shall continue to be valid and enforceable to the fullest extent permitted by law.
    

I have read this liability waiver, fully understand its terms, and voluntarily agree to its contents. I acknowledge that I am giving up substantial legal rights by signing this waiver and that I do so knowingly and voluntarily.
